If during the term of your membership you become physically unable to use a substantial portion of the health club services for 30 or more consecutive days, you may cancel your membership with a doctor’s or nurse practitioner’s certificate.
Have your doctor or nurse practitioner complete the Verification of Disability form
• Medical Disability or Death -If primary member is unable to use a substantial portion of the membership for 30 or more consecutive dates, with a completed verification of disability form (as found at Verification of Disability form
) the membership will be cancelled upon receipt. In the event of buyer’s or primary member’s death, the buyer/buyer’s estate shall be relieved of any further payments upon receipt of death certificate.
• Your effective date of cancellation for medical cancellations is the date American Family Fitness receives complete documentation (including the $100.00 relocation fee, if applicable) per cancellation polices found on your membership contract.
• In the event your membership dues or additional services were drafted after American Family Fitness receipt, a refund will be presented to your financial institution less the amount equal to exact value of services rendered.
Cancellation received on the date of membership billing will not be considered for refunds. Your effective date of cancellation will be 30-days after your last membership dues billing.
In rare circumstances in which payment has not been received for services rendered, a balance will be place on your account and an invoice will be mailed the address on file.
